Book with a Chronic Disease
Management Plan
(Referral from GP)

With a care plan, you're eligible for up to five Telehealth sessions with our dietitians each calendar year and you’ll receive a Medicare rebate of $61.80 per appointment.

Book Privately

For patients without a care plan from their GP. These appointments are

out-of-pocket and NOT covered by Medicare.

 

You might be able to claim a rebate through your private health insurance depending on your level of cover.

How to get a
Chronic Disease Management Plan

Step 1

Book an appointment with your GP

​

Let your GP know you're seeking dietitian support for a chronic condition, including:

  • Coeliac disease

  • IBS or SIBO

  • Constipation

  • Bloating

  • Type 2 diabetes 

  • High Cholesterol 

  • Hypertension 

 

If eligible, your GP will provide you with a Chronic Disease Management (CDM) plan.

Provide your GP with our referral details

Step 3

Send us your referral

Once your GP has completed the referral, you can:

  • Ask them to email it directly to us, or

  • Email it to us yourself once you’ve received it

Important: We recommend booking your consult in advance, if we haven’t received your referral prior to your appointment we are more than happy to reschedule you.​​

Cancellation policy

If for any reason you are unable to attend your appointment please let us know as soon as possible by emailing referrals@guthealthgroup.com.

 

This allows us to reschedule your session and offer the appointment to another client who may be waiting.

Please note that an $80 cancellation fee applies if less than 24 hours notice is provided.

This policy exists to respect the time our practitioners spend preparing for your session and because we often have a two week waitlist. Last minute availability means we can support clients with chronic and urgent health concerns sooner.
