Referring Your Patient to The Gut Health Group (Telehealth Dietitian)

We are a fully telehealth clinic accepting Medicare Chronic Disease Management Plan (CDM) referrals from GPs across Australia for patients requiring support with (but not limited to) chronic gut-related conditions including coeliac disease, IBS/SIBO, bloating, constipation and weight management.

Please use the following information when completing the CDM referral:

How to Send Referrals

Once completed, referrals can be sent via:

(Please include any relevant health summaries if available.)

We will contact the patient directly to arrange their telehealth appointment. Alternatively, patients are welcome to book online themselves, as long as they have their CDM referral completed and ready prior to the consultation.
