Referring Your Patient to The Gut Health Group (Telehealth Dietitian)

We are a fully BULK-BILLED Telehealth clinic accepting Medicare Chronic Disease Management Plan (CDM) referrals from GPs across Australia for patients requiring support with (but not limited to) chronic gut-related conditions including coeliac disease, IBS/SIBO, bloating, constipation and weight management.

Please use the following information when completing the CDM referral:

How to Send Referrals

Once completed, referrals can be sent via:

(Please include any relevant health summaries if available.)

We will contact the patient directly to arrange their bulk-billed telehealth appointment. Alternatively, patients are welcome to book themselves HERE, as long as they have their CDM referral completed and ready prior to the consultation.
